CSI transepithelial abutments for customized subperiosteal implants: The power of adaptation

Transepithelial abutments for ISP provide great clinical advantages.

Customized subperiosteal implants (CSI) are a highly innovative solution in the field of cranio-maxillofacial surgery, which have become an essential instrument in cases of severely atrophic maxilla in fully edentulate patients who are unsuitable for conventional endosseous implants.

Until now, the treatment of choice for the maxillary residual alveolar ridge was with zygomatic implants that, although a widely described technique with predictable results, were not free from complications.

CSI are titanium structures custom designed to adapt to the patient’s bone structure, anchored in areas with better quality bone, enabling full passive union of a fixed dental restoration to the connections of the structure with optimum distribution of the prosthetic load.

Subperiosteal implants provide a treatment alternative for patients with severely atrophic maxilla, with a lower morbidity than complex procedures with bone grafts, for a quicker recovery of function and quality of life for the patient.

Maintaining its commitment to innovation and ongoing improvement in its medical and dental solutions, Avinent thought it would be of interest to extend and improve the connections of these CSI, for greater versatility and even better results at the level of the soft tissues.

To do so, some new transepithelial abutments specific for CSI have been created, with a converging design without sharp edges that guarantees the continuity of the connection, aiding the adaptation of the mucous membranes and allowing good vascularization, to ensure long-term implant stability.

What advantages does using CSI with transepithelials provide?

Transepithelial abutments act as an interface to transmit the connection of the prosthesis at paragingival level, reestablishing the biological width and achieving an optimum seal with the mucous membranes.

This implant-prosthesis interface guarantees the screw-mounted prosthesis has good passive fit, above all in procedures with immediate load of the subperiosteal implants.

The abutments are available in different heights to be able to adapt the position of the final prosthesis to the thickness of the existing tissues, facilitating access and maintenance care.

What are these transepithelial abutments CSI like?


· Medical grade 23 titanium abutments (Ti6Al4V)

· Available in heights of 1 to 5 mm, to combine with the different heights of the CSI connection of 2, 3 and 4 mm

· Specific abutments for CSI

· Abutments anodized in gold to facilitate camouflage with the tissues

· Converging shape that allows the tissue to grow vertically, without compressing it, to create a good seal around the connection

· Universal prosthetic platform with a diameter of 4.8 mm, compatible with the Avinent line of prosthetic solutions for transepithelials

The idea was to have shorter connections that would allow us to work with transepithelial pillars, ensuring easier rehabilitation and improving soft tissue management, which is a key factor in this type of case.
